Cynergex Group uses CARRS (Collect, Analyse, Assess and Recommend) to investigate any near-miss or loss (injury to person, damage to property or commercial loss) and identify what really went wrong. We recommend actions to reduce risk and build error-tolerant defences to protect you against future incidents.

Our experienced investigators quickly recognise root causes of adverse events and identify steps to prevent recurrence and reduce risk. CARRS assists the identification of local factors and failures within the organisation and it's systems that contributed to the incident. These factors include communication, training, operating procedures, incompatible goals, change management, organisational culture and equipment.
